2019-2020 National Honors Society Selection Candidate Information Submission Form
The following form must be a typed submission. Complete the form and submit to the adviser by 1/20/20. Make sure you save a copy for yourself.
Students must indicate their interest in membership by submitting a Candidate Information Submission Form. The CIF is an application for admission; rather it is a data collection form that assists the Faculty Council in selecting students for membership in NHS.
The Candidate Information Submission Form should be filled out by the eligible student and submitted to Mrs. Prosser by January 20, 2020. The ceremony will be held tentatively on Tuesday evening, February 25, 2020. If inducted, your family will be notified.
FHS NHS Criteria:
Be a Junior or Senior in good academic standing
Must maintain a cumulative grade point average of 3.0
Have attended FHS for at least one year (prior school accomplishments will be considered)
Complete the application with pertinent information in ALL the following areas:
Scholarship* Leadership* Character* Service
Directions: Applications are available by request through email at firstname.lastname@example.org . Describe accomplishments between grades 9-12. The Faculty Council will review and assess all the information provided. Completion of this form does not guarantee induction into NHS.

HISTORICAL INFORMATION FOR FAIRPORT HARDING HIGH SCHOOL CHAPTER OF NATIONAL HONOR SOCIETY
All juniors and seniors who have a cumulative high school GPA of 3.0 or above may apply for admission to the FAIRPORT HARDING HIGH SCHOOL CHAPTER OF THE NATIONAL HONOR SOCIETY.
Application forms can be found on the school website or from the advisor for those students who meet the GPA requirement Admission is not automatic. In addition to having the required GPA, applicants must demonstrate significant accomplishment and/or participation in all areas indicated on the application form. It is necessary to have some accomplishments in each area.
All activities, accomplishments, etc. included in the application must have taken place during high school. Do not include anything that occurred before you were in high school (grades 9-12). Doing so could jeopardize your chances for admission.
Two letters of recommendation must be sent to the NHS advisor. One must be from a teacher to speak to your academic ethics or achievements.
Alcohol violations are in breach of National Bylaws, DO NOT APPLY if you have had any alcohol or drug violations.PLEASE NOTE: Honor Society is a service organization. Membership is a highly sought-after honor, but it requires a commitment to the ideals of the organization. Your acceptance of membership will indicate your willingness to participate in National Honor Society sponsored activities, service projects and meetings. To maintain membership in NHS, you must maintain your GPA, fulfill your service hour requirements, be a good leader, show good character, and abide by the National Honor Society bylaws. Failure to comply with these requirements will result in dismissal from the National Honor Society. If your behavior in school requires disciplinary action, your membership may be revoked.

Meeting: Monday, September 11, 2017 at 7:00pm - FHS Auditorium. Interested Juniors and Seniors should attend this informational meeting to learn more about joining NHS. Current NHS members will be on hand to answer questions.
